According to Ward et al. (2014), 1 in 4 Americans suffers from multiple chronic conditions (MCC)1. An increase in the number of conditions one has is accompanied by further limitations on life activities, as well as financial adversity. Some employers are implementing wellness programs to address the complex nature of these illnesses, but few are effective in addressing MCC due to their focus on lifestyle management programs over disease management. Lifestyle management programs tend to target specific issues, such as obesity or smoking habits. While beneficial to the participants, these programs are not cost-effective2, therefore unsustainable. A disease management intervention, such as the Integrated Health Advocacy Program (IHAP), differs in its ability to identify and support those who suffer from MCC.
